Passport Reservation

U.S. Passports

Passports are accepted by appointment only.  For your convenience, appointments can now be scheduled online.  No appointments will be made by telephone.

All passport applicants must appear in person, including minor children.

Before you arrive for your appointment, please make sure that you have completed all sections of your application(s) and have all of the following items:  (Otherwise it may be necessary for you to schedule an additional appointment)

  1. Completed application – filled out in BLACK INK ONLY.
  2. Original Certified Birth Certificate or U.S. Naturalization Certificate
  3. Passport Photo – we do not take the photos here.  Photos must be 2 X 2 in size with a white or light colored background.  Please refer to www.travel.state.gov for additional information regarding photos.
  4. U.S. Government I.D. – such as a state issued Driver’s License (if out of state, you will need two (2) photo I.D.’s)
  5. Fees – Two separate fees will need to be paid. One (1) Check or Money order made payable to the U.S. Department of State AND One (1) Cash, Visa Debit/Credit Card (processing fee will apply), Check or Money Order made payable to the Henry County Probate Court.

DO NOT SIGN THE APPLICATION UNTIL AN AUTHORIZED AGENT INSTRUCTS YOU TO DO SO.

Please see below for additional information.

The following requirements are necessary to apply for a U.S. Passport Book and/or Passport Card, or for the renewal of a passport book for anyone who was under the age of 16 when the passport was applied for:

  • If you were born in the United States, you will need one of the following: a) A certified copy of your birth certificate (from Vital Records, not a hospital); b) A previous United States Passport. Please note that your certified birth certificate must be mailed in with your application. However, it will be returned to you once your passport has been issued by the U.S. Department of State. For all minors, you must submit a birth certificate that includes parental information even if they have a valid or expired passport (please bring the passport with you if you still have it).
  • If you were born outside the United States or the U.S. Territories, Naturalization papers or a previous U.S. Passport are required. Please note that your documents must be mailed in with your application and will be returned when your passport has been processed.
  • One (1) 2”X2” passport photograph on a white or very light colored background.
  • A valid driver’s license, government ID card or an ID card obtained from the Division of Motor Vehicles must be presented as picture ID.
  • Applicants who are under the age of 16 must be accompanied by both parents to apply for a passport.  However, if only one parent is available to apply, you will need to fill out form DS-3053 (Statement of Consent or Special Circumstances: Issuance of a Passport to a Minor Under Age 16) and submit with the minors passport application.  Applicants who are between the ages of 16 and 17 must be accompanied by at least one (1) parent named on the birth certificate. 
  • Two (2) separate forms of payment are required. (Please note - We do not accept Debit/Credit cards for the US Department of State’s fee).

Fees for the PASSPORT BOOK are as follows: (Estimated time of return is approximately 6 weeks):

  • Age 16 and over (valid for 10 years) - $110.00 (Check or Money Order) Made payable to the U.S. Department of State. $25.00 (Cash, Check, Money Order, MasterCard or Visa debit/credit cards only) Made payable to the Henry County Probate Court.
  • 15 and under (valid for 5 years) - $80.00 (Check or Money Order) Made payable to the U.S. Department of State. $25.00 (Cash, Check, Money Order, MasterCard or Visa debit/credit cards only) Made payable to the Henry County Probate Court.

Fees for the PASSPORT CARD are as follows (NOT VALID FOR AIR TRAVEL):

  • Passport Cards may only be used for land and sea travel between the U.S. and Canada, Mexico, Bermuda and the Caribbean. (It cannot be used for travel by air)
  • Age 16 and over (valid for 10 years) - $30.00 (Check or Money Order) Made payable to the U.S. Department of State. $25.00 (Cash, Check, Money Order, MasterCard or Visa debit/credit cards only) Made payable to the Henry County Probate Court.
  • Age 15 and under (valid for 5 years) - $15.00 (Check or Money Order) Made payable to the U.S. Department of State. $25.00 (Cash, Check, Money Order, MasterCard or Visa debit/credit cards only) Made payable to the Henry County Probate Court.

Fees to apply for both, PASSPORT BOOK AND PASSPORT CARD, at the same time are as follows:

  • Age 16 and over (valid for 10 years) - $140.00 (Check or Money Order) Made payable to the U.S. Department of State.
  • $25.00 (Cash, Check, Money Order, MasterCard or Visa debit/credit cards only) Made payable to the Henry County Probate Court.
  • Age 15 and under (valid for 5 years) - $95.00 (Check or Money Order) Made Payable to the U.S. Department of State
  • $25.00 (Cash, Check, Money Order, MasterCard or Visa debit/credit cards only) Made payable to the Henry County Probate Court.

NOTE - If you need to EXPEDITE the processing of your Passport Book or Card, an additional fee of $60.00 per application will be required. Additional overnight postage fees may be required. Please contact the Henry County Probate Court for further details.
